Approximately 90 days prior to your license expiration, board staff will mail your renewal form.

Licensees requesting to reactivate their license from inactive or retired status are required to pay additional fees and comply with specific continuing education requirements. Please contact the board office at MQA.Chiropractic@flhealth.gov to request your reactivation requirements.

The department will process your renewal request upon receipt of:

  • Completed renewal form
  • Required renewal fees
  • Verification of chiropractic license
  • Letter of acknowledgment from dean of appointing college

Have you registered for an account and linked your license in the new MQA Online Services Portal?

No, I have not registered for a new account

  1. Click the Renew Online button.
  2. Select No and click Register to go to the main sign in screen.
  3. Under New User, select Register Now to begin the registration process.
  4. Follow the prompts to create your new account.
  5. After you Sign In with your new user ID and password, you will then be prompted to add your license to your account before you can renew. You will need four (4) items:
    1. Your license number
    2. Your social security number
    3. Your date of birth
    4. Your mailing address zip code on file with the Department of Health (Please note that your mailing address zip code can be found on your renewal postcard or your physical license)
  6. Once you have successfully linked your license to your account, you will see the Manage My License section on your dashboard.
  7. During renewals, a “Renew My License” message will populate no later than 90 days prior to your license expiration date (Note: If you do not see the “Renew My License” message, please check back at a later time). Under Renew My License, select Renewal Application to submit your renewal application and payment.

Yes, I have registered for an account, but I have not added my license

  1. Click the Renew Online button.
  2. Select Yes to go to the main sign in screen.
  3. Enter your user ID and password you selected during the one-time registration process, then click Sign In.
  4. Once on your account, you must select Add My License or Previous Application to add your license. You will then be prompted to add your license to your account before you can renew. You will need four (4) items:
    1. Your license number
    2. Your social security number
    3. Your date of birth
    4. Your mailing address zip code on file with the Department of Health (Please note that your mailing address zip code can be found on your renewal postcard or your physical license)
  5. Once you have successfully added your license to your account, you will see the Manage My License section on your dashboard.
  6. During renewals, a “Renew My License” message will populate no later than 90 days prior to your license expiration date (Note: If you do not see the “Renew My License” message, please check back at a later time). Additional Information:
You may renew online if you have a credit or debit card to complete the transaction. Please note that you can now renew online, print a copy of your application summary and mail it with your cashier’s check or money order to:

At least 90 days before your license expires, the department will mail you a renewal notification postcard to your last known mailing address of record. You will have until midnight, Eastern Time, on the day your license expires to renew. Failure to renew an active or inactive license by the expiration date will result in the license being placed in delinquent status. Failure by a delinquent licensee to renew before the expiration of the current licensure cycle renders the license null and void without any further action by the board or the department.

Approximately 90 days prior to your license expiration date, the board office will mail you a copy of your renewal application letter.

In order to renew your Chiropractic faculty certificate, you must complete and submit the renewal application along with your fee.

Note: If your state is able to verify your license electronically, please have your verification sent to MQA.Chiropractic@flhealth.gov

Upon receipt of all required documentation, board staff will review and process your request. If any additional information is required you will be notified.

Name Changes

Name changes require legal documentation showing the name change. Please submit a request including your full name as it appears on your license, profession, license number, your new name, your date of birth, the last four digits of your social security number, and your signature. Attach supporting documents, which must be one of the following:

  • A copy of a state issued marriage license that includes the original signature and seal from the clerk of the court
  • A divorce decree showing the name change
  • A court order showing the name change (adoption, legal name change, federal identity change)

Any one of these will be accepted unless the department has a question about the authenticity of the document. A social security card is not considered legal documentation.

Certified check or money order should be made payable to the Florida Department of Health.

Renewal Fee$105.00

Licenses not renewed by the renewal deadline become Null & Void1.

By submitting the appropriate renewal fees to the department, a licensee acknowledges compliance with all requirements for renewal.
