Applying for a License

Florida passed House Bill 975 following the 2024 legislative session, which requires this profession to complete electronic fingerprinting. Your application cannot be approved until this requirement, along with all other licensure criteria, has been met. The requirements for licensure as a Chiropractic Physician are as follows. Applicants must:

Be at least 18 years of age.

Successfully complete the National Board of Chiropractic Examiners certification examination in Parts I, II, III, and IV, and the physiotherapy examination, in addition to the Florida Laws and Rules Examination*, with a score approved by the board.

In order to qualify to sit for the National Board of Chiropractic Examiners certification examination the applicant must meet the following requirements:

  • Have graduated from a chiropractic college that is accredited by or has status with the Council on Chiropractic Education or its predecessor agency.
  • For applicants who were enrolled at a chiropractic college prior to July 2, 1990: Have completed at least two years of residence college work, consisting of a minimum of one-half the work acceptable for a bachelor’s degree granted on the basis of a 4-year period of study, in a college or university accredited by an accrediting agency recognized and approved by the United States Department of Education or have produced a credentials evaluation report from a board-approved organization that deems the applicant’s education equivalent to a bachelor’s degree.
  • For applicants who were enrolled at a chiropractic college after July 1, 1990: Have been granted a bachelor’s degree, based upon four academic years of study, by a college or university accredited by a regional accrediting agency which is a member of the Commission on Recognition of Postsecondary Accreditation or have produced a credentials evaluation report from a board-approved organization that deems the applicant’s education equivalent to a bachelor’s degree.
  • Applicants who were enrolled at a chiropractic college after July 1, 2000: Have been granted a bachelor’s degree from an institution holding accreditation for that degree from a regional accrediting agency which is recognized by the United States Department of Education or have produced a credentials evaluation report from a board-approved organization that deems the applicant’s education equivalent to a bachelor’s degree.
  • The applicant’s chiropractic degree must consist of credits earned in the chiropractic program and may not include academic credit for courses from the bachelor’s degree.

*The Florida Laws and Rules examination is no longer administered by the Department of Health. This examination is administered through the National Board of Chiropractic Examiners (NBCE). The website is http://www.nbce.org/ and the contact number is 1-800-964-6223.

In accordance with Rule 64B2-11.001, Florida Administrative Code (F.A.C.), credentials evaluation reports that demonstrate an applicant’s undergraduate education to be equivalent to a bachelor’s degree may be accepted by the following board-approved organizations:

  • Josef Silny & Associates International Educational Consultants;
  • Foundation for International Services, Inc.;
  • Education Credential Evaluators;
  • International Consultants of Delaware, Inc.;
  • Center for Applied Research, Evaluation & Education, Inc.;
  • Foreign Academic Credentials Services, Inc.;
  • World Education Services, Inc.;
  • World Education Services SE Branch; and
  • Any current member of the National Association of Credential Evaluation Services

Applicants with Health History

If a “Yes” response is provided to any of the questions in the health history section, provide the following documents directly to the board office:

A letter from a Licensed Health Care Practitioner, who is qualified by skill and training to address the condition identified, which explains the impact the condition may have on the ability to practice the profession with reasonable skill and safety. The letter must specify that the applicant is safe to practice the profession without restrictions or specifically indicate the restrictions that are necessary. Documentation provided must be dated within one year of the application date.

A written self-explanation, identifying the medical condition(s) or occurrence(s); and current status.

Applicants with Discipline History

Applicants with prior disciplinary actions are required to submit the following:

Board Actions – Certified copies of document(s) relative to any disciplinary action taken against any license. The documents must come from the agency that took the disciplinary action and must be certified by that agency.

Self-Explanation – A detailed description of the circumstances surrounding your disciplinary action and a thorough description of the rehabilitative changes in your lifestyle since the time of the disciplinary action which would enable you to avoid future occurrences. It would be helpful to include factors in your life, which you feel may have contributed to your disciplinary action, what you have learned about yourself since that time, and the changes you have made that support your rehabilitation.

Applicants with Criminal History

Applicants with prior criminal convictions are required to submit the following:

Final Dispositions/Arrest Records – Final disposition records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. If the records are not available, you must have a letter on court letterhead sent from the clerk of the court attesting to their unavailability.

Completion of Probation/Parole/Sanctions – Probation and financial sanction records for offenses can be obtained at the clerk of the court in the arresting jurisdiction. Parole records for offenses can be obtained from the Department of Corrections or at the clerk of the court in the arresting jurisdiction. If the records are not available, you must have a letter on court letterhead sent from the clerk of the court attesting to their unavailability.

Self-Explanation – Applicants who have listed offenses on the application must submit a letter in their own words describing the circumstances of the offense. Include in your letter the date of the original offense, the charge, and the jurisdiction where it occurred.

Section 456.0635, Florida Statutes (F.S.), provides that health care boards or the department shall refuse to issue a license, certificate or registration and shall refuse to admit a candidate for examination if the applicant:

  1. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under Chapter 409, F.S., (relating to social and economic assistance), Chapter 817, F.S., (relating to fraudulent practices), Chapter 893, F.S., (relating to drug abuse prevention and control) or a similar felony offense(s) in another state or jurisdiction unless the candidate or applicant has successfully completed a drug court program for that felony and provides proof that the plea has been withdrawn or the charges have been dismissed. Any such conviction or plea shall exclude the applicant or candidate from licensure, examination, certification, or registration, unless the sentence and any subsequent period of probation for such conviction or plea ended:
    1. For the felonies of the first or second degree, more than 15 years from the date of the plea, sentence and completion of any subsequent probation;
    2. For the felonies of the third degree, more than 10 years from the date of the plea, sentence and completion of any subsequent probation;
    3. For the felonies of the third degree under section 893.13(6)(a), F.S., more than five years from the date of the plea, sentence and completion of any subsequent probation;
  2. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under 21 U.S.C. ss. 801-970 (relating to controlled substances) or 42 U.S.C. ss. 1395-1396 (relating to public health, welfare, Medicare and Medicaid issues), unless the sentence and any subsequent period of probation for such conviction or pleas ended more than 15 years prior to the date of the application;
  3. Has been terminated for cause from the Florida Medicaid program pursuant to section 409.913, F.S., unless the candidate or applicant has been in good standing with the Florida Medicaid program for the most recent five years;
  4. Has been terminated for cause, pursuant to the appeals procedures established by the state or Federal Government, from any other state Medicaid program, unless the candidate or applicant has been in good standing with a state Medicaid program for the most recent five years and the termination occurred at least 20 years before the date of the application;
  5. Is currently listed on the United States Department of Health and Human Services Office of Inspector General’s List of Excluded Individuals and Entities.

Applicants for initial licensure must use an approved Livescan Service Provider to submit electronic fingerprints to the Florida Department of Law Enforcement (FDLE) for the purpose of conducting a search for any Florida and national criminal history records that may pertain to applicant. The results of the search will be returned to the Care Provider Background Screening Clearinghouse and made available to the Department for consideration during the licensure process. The fingerprints submitted by the applicant will be retained by FDLE and the Clearinghouse. All costs for conducting a criminal history background screening are borne by the applicant.

It is important to use the correct Originating Agency Identification (ORI) when submitting fingerprints. If you do not provide an ORI number or if you provide an incorrect ORI number to the Livescan service provider, the board office will not receive your fingerprint results.

The ORI number for the Board of Chiropractic Medicine is EDOH2016Z.

The applicant is fully responsible for selecting an approved Livescan Service Provider and for ensuring the results are reported to the Department. Print this electronic fingerprinting form and take it with you to a Livescan Service Provider.

Within 7-14 days of receipt of your application, the board office will notify you of the status of your application and any remaining required documentation that needs to be submitted.

Once you have submitted your application, you can check the status online. Select “Status” from the menu at the top right-hand side of the page.

The following documentation is required to complete your application:

  1. Official National Board of Chiropractic Examiners- Contact the National Board of Chiropractic Examiners (NBCE) to request your official National Board scores to be submitted directly to the board office. Visit the NBCE at http://nbce.org/ for further information on requesting score reports.
  2. Official Final Transcript- Have your official final transcripts mailed directly from your college or university to the board office at:

  1. Official License Verification- If you are licensed or have ever held a license in another state, contact that state’s licensing office and request for license verification to be sent directly to the Board of Chiropractic Medicine.
  2. If you responded “Yes” to any of the Health History questions on the application, submit a letter to the board office. The letter must provide relevant dates and circumstances of your treatment and/or addiction and include the names and addresses of the medical practitioners or hospitals that provided your treatment.
  3. If you responded “Yes” to any of the Discipline History questions on the application, contact the state board where the discipline occurred to request that certified copies of the board order and any other related documentation be submitted directly to the board office.\

You must also submit a letter in your own words describing the circumstances of the offense(s). Your letter must include the date(s) of the original offense(s), the charge(s) and the jurisdiction where it occurred.

  1. If you responded “Yes” to any of the Criminal History questions on the application, contact the Clerk of the Court in the jurisdiction in which the offense occurred and request that a certified copy of your final/official court disposition be mailed directly to the board office. If the records are unavailable, you must have a letter on court letterhead sent from the Clerk of the Court attesting to their unavailability.

If applicable, request the following documentation to be sent directly to the board office: a certified copy of your completion of probation and documentation showing that you have paid all fines. If the records are unavailable, you must have a letter on court letterhead sent from the Clerk of the Court attesting to their unavailability.

You must also submit a letter in your own words describing the circumstances of the offense(s). Your letter must include the date(s) of the original offense(s), the charge(s) and the jurisdiction where it occurred.

Note: All applications with “Yes” responses to the history questions on the application will be presented to the board for review. Board staff will notify you of the date that your application will be presented in the event that you would like to attend.

Applying for License by MOBILE Endorsement

To be licensed as an Chiropractic Physician in Florida through the MOBILE Endorsement pathway you must meet the following qualifications, as required by Section 456.0145, Florida Statutes:

    1. Must hold an active, unencumbered license issued by another state, the District of Columbia, or a territory of the United States in a profession with a similar scope of practice, determined by the board or the department, as applicable.
    2. Must have obtained a passing score on a national licensure examination or hold a national certification recognized by the board;

    OR

    An applicant for a profession that does not require a national examination or national certification is eligible for licensure if the applicable board, or the department if there is no board, determines that the jurisdiction in which the applicant currently holds an active, unencumbered license meets established minimum education requirements and, if applicable, examination, work experience, and clinical supervision requirements are substantially similar to the requirements for licensure in that profession in this state.

    1. Must have actively practiced the profession for at least two years during the four-year period immediately preceding the date of submission of this application.
    2. Must not have ever been reported to the National Practitioner Data Bank, unless the applicant successfully appealed to have the report removed or if the reported adverse action was a result of conduct that would not constitute a violation of any Florida law or rule.
    3. Must not be the subject of a disciplinary proceeding in a jurisdiction in which he or she holds a license or by the United States Department of Defense for reason related to the practice of the profession for which the applicant is applying.
    4. Must not have had disciplinary action taken in the five years immediately preceding the date of submission of the application.

    Refer to section 456.0145(2)(c), Florida Statutes, for licensure ineligibility criteria.

    Applicants must complete the following steps:

    1. Contact the state board(s) in which a license has been held, to request License Verification be submitted to the Florida board office. If the state has an online verification including disciplinary actions, the online verification may be acceptable.
    2. Contact National Board of Chiropractic Examiners (NBCE) to request your passing National Examination score transcript be released to the board office.
    3. Register for and pass the Florida Laws and Rules Examination through National Board of Chiropractic Examiners (NBCE).
    4. Complete fingerprinting through an approved Livescan provider after registering with the CHAI system. Ensure you have the ORI number: EDOH2016Z.
    5. Request verification of the required Malpractice insurance to the board office.
    6. Apply online at www.flhealthsource.gov, pay applicable fees, and upload any supporting documentation.
    7. If applicants have any affirmative answers in the Criminal, Personal, or Disciplinary sections of the application, submission of the personal statement and supporting documentation can be uploaded with the online application.
    8. Please allow up to 30 days for initial review of new applications. All applications and document submissions are reviewed in the order they are received. After your application is reviewed, any deficiencies will be communicated to you in writing by our Board staff. For timely processing, please submit all required supporting documentation with your application and the requisite fees. If you are having documentation submitted by another entity on your behalf, please verify the email and/or mailing address to ensure delivery to the board office.
    9. When all requirements are met the license will be issued to qualified within 7 days pursuant to Section 456.0145(2)(e), Florida Statutes.

    Must hold a valid/current license to practice chiropractic medicine in the state of Florida

    Complete a 100-hour course in acupuncture provided by a college or university which is recognized by an accrediting agency approved by the United States Department of Education.

    The Board adopts a passing score as set by the National Board of Chiropractic Examiners.

    Initial License—Dry Needling Certification

    Effective June 26, 2024, Chiropractic Physicians must be certified to perform Dry Needling. To qualify for Dry Needling Certification you must submit documentation to verify you meet the following requirements, listed in section 460.4085, Florida Statutes:

    1. Completion of 40 hours of in-person continuing education on the topic of dry needling for Chiropractic Physicians not certified in Chiropractic Acupuncture and 24 hours of such in-person continuing education for Chiropractic Physicians certified in Chiropractic Acupuncture, and passage of a written and practical examination. Online or distance-based courses do not qualify as approved hours to meet the Dry Needling Certification requirements.
      Course content must be approved by one or more of the following entities:
      • An entity accredited in accordance with s. 460.408
      • The board (verify course approval through CEBroker)
      • The American Chiropractic Association
      • The International Chiropractic Association
      • Providers of Approved Continuing Education (verify course approval through PACE)
      • The American Medical Association
      • The American Osteopathic Association
    2. Completion of at least 10 patient sessions of dry needling performed under the supervision of a licensed chiropractic physician, allopathic or osteopathic physician, or physical therapist holding a Doctor of Physical Therapy degree who has actively performed dry needling for at least 1 year. A chiropractic physician must provide satisfactory documentation to the board demonstrating that he or she has met the supervision and competency requirements of this paragraph and does not need additional supervised sessions to perform dry needling.

    At the request of a licensee, the board may do any of the following:

    • Review coursework completed before July 1, 2024, to be approved to satisfy the coursework requirements of this section.
    • Waive some or all of the hours or requirements of subsection (1) if the licensee presents satisfactory proof of completing coursework that constitutes adequate training of dry needling or of the components of education and training required for dry needling.
    • Determine whether the licensee has received adequate training to be eligible to perform dry needling.

    Review sections 460.4085, Florida Statutes and 64B2-17.004, Florida Administrative Code for more information regarding Dry Needling Certification for Chiropractic Physicians.

    The following steps apply to individuals who hold a Florida Chiropractic Physician license, that is active and in good standing and wish to obtain the Dry Needling Certification:

    • Submit the application for Dry Needling Certification.
    • Submit documentation to verify completion of adequate education and training to be eligible to perform dry needling.
    • In addition to the continuing education/training requirements specified in the Florida Statutes and Florida Administrative Code, the documentation must include a determination by the instructor that the Chiropractic Physician being trained in dry needling demonstrates the requisite competency and psychomotor skills to safely perform dry needling, and has completed 10 patient sessions of dry needling under one of the following circumstances:
      1. While licensed as a Chiropractic Physician in another state or while serving as a Chiropractic Physician in the United States Armed Services; or
      2. Under supervision as defined herein, where the supervisor has documented that the supervised physician has met the supervision and competency requirements and needs no additional supervised sessions to perform dry needling. It is the responsibility of the supervised physician to maintain all documents.
    • Pursuant to s. 460.4085, F.S., an applicant may provide proof of completion of coursework that substantially complies with the training requirements established by the Board, which may be used to satisfy the hourly requirements of Section 460.4085(1)(a), F.S. on an hour per hour basis.  However, in no case may the requirement to pass a written and practical examination be waived.

    A fee is not required to apply for this certification.

    Processing Times

    Florida law provides that an initial application must be reviewed within 30 days.
